Is it extrem focal-length
or some special equipment he uses?

He used Clairmont's Swing & Shift lenses in combination with Zeiss Ultra Primes.

This is the Arri system, which is similar to the Clairmont system. If you download the instruction manual at the bottom of the page, there are examples of how it works:
