Why is diffusion often placed behind the lens and not infront? Is there a specific type of diffusion this is done with- what about about other effect filters?

Only net diffusion (like stretched pantyhose material) is usually placed behind the lens. Glass diffusion is usually place in front of the lens, except for extreme telephoto lenses (and usually no diffusion is used then.)

---Glass behind the lens will shift the focal plane back.

With an extreme telephoto, the shift isn't enough to be all that significant because of the greater 'depth of FOCUS', cnot to be confused with depth of field.
