Lens Collimation/Repair house recommendations...

So, the director I'm working for has a Frezzolini LW-16 that we're going to shoot with.
He said the lens that came with is a "c-mount" Angenieux 12-120 zoom type lens, but that at wider f-stops (don't know the specifics yet) there tends to be some blurring at the edges of the frame. Also, there's a very good chance that the focus is out of phase with the focal length markings.
I'm not too concerned about the focus issue, I can just compensate for that by getting critical...however, the blurred edges is the big issue.
Can anyone recommend a good place to get it repaired, or if there's a simple DIY method, I'd love to hear it!
Thanks in advance!
