Any edition of the American Cinematographer Manual will have a depth of field chart. Keep in mind that the format doesn't effect the depth of field of your lens. The format effects the field of view. Say a "normal" field of view is about 40 degrees. You would need a 35MM lens in the 35MM format; a 17MM in the S16 format; or a 12MM in the 2/3" format to get a 40 degree field of view. So if you are using a 12MM lens in the 2/3" format you will look up your f stop in the All Formats Depth of Field Table under 12MM.

The public library usually has a copy of the American Cinematographers manual, if you want to do it on the cheap, just photocopy what you need.
