One shoot I operated boom for two takes, but the lady operating the Nagra said she heard squeaking coming from my shoes.

How common is that?

Sound recordists pick up all sorts of sounds, in one case it wasn't the shoes, but my trouser  (pants in US) legs rubbing together when doing a slow tracking shot with a Steadicam.

Interesting.  It was years ago.  When she came up to me and told me I half didn't believe it.  I was wearing Reeboks at the time.  I wonder if that had anything to do with it.
