I have been PAing on a few reality TV gigs recently, and the crews have always been awesome. I worked my ass off, and they all told me to keep in touch, but so far I've only contacted one DP who gave me an awesome letter of recommendation.

What is the best way of reaching out to other past fellow crew members?

It's probably better to keep in touch with the crew members directly above you, as they'll be the ones most likely to hire you again. As for how to reach out, well, meeting for beers usually works.

I would love to meet for beers, and we did quite a bit after shooting, but we are spread out all over the country.  I was local crew and they flew in.
